The Healthcare Passport for patients with eating disorders has been created in collaboration with Experts by Experience from the EmpowerED Adult Eating Disorders Lead Provider Collaborative.

It a place to ensure reasonable adjustments are made for patients with eating disorders, and to support an understanding of the ways that an eating disorder can make it difficult for people to be at their best in a range of healthcare settings. 

The Healthcare Passport is a document held by the patient. It is their responsibility to keep this up to date. Patients receive access to a patient guide and video guide to support them to complete the Healthcare Passport.
